Chris, 5th wheel is what the "big rigs" have, gooseneck is what you see on trailers that arch "gooseneck" over the tail gate of pickups, attach in the bed of the truck.

In my neck of the woods, Goosenecks are usually pulled by "Doolies'.
Most guys prefer a one ton 'Dooley' with Diesel power for towing.
