NOS GM Chevrolet dealer special order form. Made by GM back in 1966. These are a three piece form that has the carbon copy in the middle.

When you went into a dealer to special order your car, then the salesman would fill this out. The top part would be sent to the Chevrolet Zone office and the dealer would keep the back green copy.
