John Z has stated the earliest MVSS labels self-adhered but quickly fell off. The fix was the mylar overlay. I looked at an untouched 09C COPO a few years ago and the label and overlay were still attached so the fix worked well.

The label and its location are depicted in the '69 AIM UPC 0 page B1 but no date.

The label was required for cars built for the 1970 model year; those seem to be cars with re-set body numbers and 08A tag dates. The last July 1969 VIN produced according to GM was N669119. However when production resumed August of '69, there must have been released orders at Fisher because there are later N669xxx VINs with 07A tags. Those cars may have gotten the revised door but no label.
