Joe, That is a 772 fan. GM used 2 different suppliers for these fans. One has the part # and date code, the other does not have any part # or date stamp. If you remove the clutch from this fan it should have an H and the word FRONT stamped on the hub section under the clutch. Fairly common actually and because of no date code can be used on anything that would have used a 772 fan. Not sure on the CH clutch usage, but that style clutch was primarily pre 1970.
