This is exactly why I posted these pictures to begin with.
We do believe it is original to the car. This is the second one I have seen like this. I believe Joe Barr is correct.

Perhaps this is the same reason your 1969 COPO Chevelle did not have "Ribbed" rear ash trays? They ran out of stock on the assembly line

Dan, what is the build date/plant on that car? If that is an original script then it would make sense to it being a very early production car.

I owned a 69 Chevelle Malibu convertible that came with the `68 design dash plaque also. It was built in April of `69 and it had to be the original piece as the whole car was untouched.
