Bill,

I believe the correct fastener for '69 is a large pan-head screw. It also had either a lock washer or captured external tooth star washer. I can check for sure on Sunday to be sure.

Incidentally, the gent that worked in the Baltimore factory told me that during the 69 production year, Chevrolet started including wooden-handled phillips screw drivers with the jack assembly so the customer could take off the SS cap with a tire change and be able to re-secure the wheel in the trunk. I don't know if all the factories did the same or not.
