WOW ! LOVE THE CATALOG....
IM Looking for one just like that up to 1972.
That 1570 plate looks just as I described earlier ....69 issue I believe.
BB had the large starter nose hole knockout. SB Had it too but on the inside for a 10 1/2" clutch.
Those were used for a few years then went to the 3 hole design, then added a much larger crank hole with less outer holes for attachment and less material on the outer diameter like the one Sam has pictured above yours.
The one you have is Perfect for a '68 on up Car assuming it was added new from Lakewood. (the extra lower holes I believe are there for use with other GM shields.)
Those are extremely rare today.
I have this style on my '69 SB 302 w/ Doug Nash M22 2:52 Muncie.
